Sweet Memory in Singapore with Ayek n Bella...- 17.01.2008























Completed ED run!!!

Dear Blog,

Yesterday was my last shift for ED run. Doing ED run is really awesome. But actually, it depends on our luck too.What I meant ; it depends on the registrars that we attached to and the cases that happened in that particular time. For me, the last 2 shift were pretty boring.Nothing much happened and the registrars were not too cool. boo...booo...

Yesterday shift was so damn busy until all beds in ED and APU were full occuppied. But still, a lot patients came in until one of ED consultants has to put a notice at the ED registration centre *Our department is overwhelming by patients, so waiting time might be too long*. Since it was so busy, I felt quite lost and being abandoned by my registrar and suprisingly, my registrar did notice about it. He said sorry for that but I said that's ok since I do understand the situation. To be truth, I attended the session just for the sake of my logbook, need someone to sign off the attendance since the passing criteria is at least; have to attend for 6 sessions..hahahaha.*that's so bad jiha- not a good example to follow*

The only things that I can consider as interesting cases are like pneumonia ,PV(per vaginal) bleeding and reduction of Collie's fracture. I was so happy because I can really hear the crackles at the basal of the left lung of the patient and when my registrar asked me about the findings, I answered confidently.hahahaha.*clap clap*

About the PV bleeding, the patient is 3/52 post-partum and suddenly she has excessive bleeding coming from her vagina. Because of the massive bleeding, the my consultant rang the Gynae Registrar and like what expected, the she did the PV examination. She has to do it twice since the vaginal way was obstructed with huge amounts of fresh blood and blood clots. And this became possible after they used the suction machine to suck all the blood that coming out continuously from inside. Eventually, we can see the patinet's cervix and apparently, the bleeding was not from the cervical wall. It might be from the uterus, the hypothesis that made by the Gynae Registrar and the patient was transferred immediately to the operation theathre to find out the cause of the bleeding.- this is my 1st time seeing real human cervix and I'm so jakun with that because beforeb this, we can just see the dummies' cervix. hahahaha.

And for the Collie's fracture, the procedures involved was damn cool. Oppss...and just for revision, Collie's fracture is the extra-articular fracture of distal radius and in this case, the fracture presented with dorsal angulation. This procedures involved the reduction of the fractured radius, applying and splitting the Plaster of Paris a.k.a cast.

In summary, Alhamdulillah, I've completed the ED run and now I'm looking forward for Locomotor run. Yeah, another good news, one of my IMU friend, Siti Nurliana a.k.a Ayek is here. She came all the way from Wellington tu.yeah..yeah..jumpe ayek!!!hahahahah..

Alhamdulillah...

Dear Blog,

I'm so happy. I've completed all the topics which need to be signed-off by registrars in ED logbook. Alhamdulillah. Yesterday, I've got a major trauma case which I kept on praying that I can get to see once since last weekends. Alhamdulillah.Sesungguhnya, Allah telah mengkabulkan doa saya. Alhamdulillah byk2. After this, I just need to come for another 2 sessions without any worry and I'll do my best to gain more experiences. However, I feel quite frustated because I didn't get any distinction for any topic although during the discussion, I can say that I did well by answering most of the questions that given by the registrars. But that feeling faded, after yesterday's tutorial session. The tutorial session was conducted by Dr.Andy who is one of ED Consultants. He is also coordinator of Auckland ED Run. He said, don't bother about getting distinction because the parameter is varied. It totally depends on the individual and most of the registrars are quite new and they don't know how deep should the students know about that particular topics. So don't be disappointed. After that, I feel much more relief. Furthermore, I'm just hoping to pass and never aim for distinction for the overall ED run. That's it. Hahaha.lol.

Yesterday session ended at 11.30pm but my registar is so kind.She allow me to go home earlier. So I went home around 10.50pm and reached at 11.15pm.hahahahaha.But today morning, I have to wake up earlier since I had MSE meeting at 8.30am.That's so terible because I didn't have enough sleep and rest. But today's topic is so damn interesting. It's about 'SEPSIS'. So I didn't fall asleep throughout the session.*clap clap*

After that, I went to Lim's to buy some groceries. But I'm not that lucky since the weather is so bad, like cat and dog rain.hahaha..means heavy rain. hahahaha.Can't believe it!! I'm using idiom?? hahahaha.lol. Frankly speaking, I need to brush up and improve my English especially for the communication skill. Really have to strugle for that. huhu.

Ok.I need to stop here. Start to feel sleepy.But still have a lot of things to be revised. Huhu. Jiha...aja aja fighting!!!

Happy Birthday Ayah!!!




Dear Blog,

Today is my dad's 52nd birthday. I feel really..really..sad because he's not with us anymore. Usually, I'll be the 1st person yang ucapkan "Selamat Hari Jadi Ayah". And like usual, my dad will say "terima kasih, awak la org pertama yg wish ayah..org lain tak ingat kot".hahaha.It's kind like a routine every 22/06, aku akan call ayah pagi2 utk membuat ucapan tu.Tetapi,sesungguhnyer mulai tahun ni, aku tidak akan berpeluang utk melakukannyer lagi. Aku sangat sedih. Hatta ketika menulis blog ni pun mataku berlinangan.Ayah, jiha sangat merindui ayah. Walaupun, ianye telah berlalu selama 3 bln 5 hari,aku masih merasainya. Apa yg mampu ku lakukan hanya menghadaiah sepotong surah Yassin n tahlil utk ayahku. Ya Allah, Kau tempatkanlah arwah ayahku dikalangan para solihin. Amin.

Ayah, jiha akan belajar bersungguh2 utk mencapai cita2 ni kerana jiha tau yg ayah sangat bangga apabila jiha membuat keputusan utk melanjutkan pelajaran dlm bidang perubatan. InsyaAllah, I'll be the 1st doctor in our family. I'll take care of our family.I'll make you proud of me. InsyaAllah.

Yeye..2 days break!!!

Dear Blog,

I'm so happy because I got 3 signed-off for my logbook for today.Yeye.*clap clap*lol. Special credit to my dedicated registrar.hahahahaha.He said I'm so enthusiastic.hahahahaha.The fact is I really wanna complete my logbook ASAP and after this I don't want be bother about it. Today's tutorial on Oxygen Therapy was cancelled.huhu.Really make me frustated. I'm already prepared for that.hahahaha.Just kidding!!!lol

Tomorrow is Saturday, means I can wake up late.hahahaha.Really love the weekends.But still wondering what am I going to do for this weekends.Study perhaps???hahahaha..

Need to stop here.I'm so damn hungry.Wanna have my dinner.Nyum..nyum..

Enjoying Emergency Run??? 1st experience having patient died...

Today I'm free since I had evening shift for yesterday.What a relief!! hahaha. I really like this kind of thing.*clap clap*

Yesterday, my shift ended at 11.30 pm. That session was so..so busy...I just can't believe how tought an ED doctors have to be..MasyaAllah..To be frank, I do enjoy doing ED, I can learn a lot things especially we have to be fast and it's quite difficult at the beginning. However, I'm getting used with the fast-pace-in-making decision and treatment. Yesterday, I had a difficult situation..My patient was pronouced death and this is my 1st experience facing it. I never thought he will be ended up like that. He was brought in by the ambulance.He collapsed on the soccerfield and he didn't have any external injury. Even, his GSC(Glasgow Coma Scale) is 8 which is not that bad. However, when he started to vomit out blood, I started to think there's something wrong.Furthermore,it was not ordinary vomiting but it's more like projectile vomiting. The blood was projectected with high velocity and it hit one of the in-charge nurses. Thank God..I made a quick move and it didn't hit me or else surely my white tudung will be turn into spotted dark red. My team has done a great work..they performed endotracheal intubation although it's hard to get that thing into his trachea since he kept on vomiting. We have to keep on doing suction to prevent from aspiration into the lungs. Oooh..I forget to mention his vital signs was tremedously made me shock..he's afrebrile, bradycardia and his BP was 220/175..it's so high...n my team suspected his having cerebral bleeding...so once his condition become quite stable, we sent him for CT scan..and the result was what they expected ..he's having massive cerebral haemorrhage involving left basal ganglia haemorrhage with right midline shift(line where the separation of hemispheres take place). My consultant contacted the on-call neurosurgeon consultant for second opinion and he agreed with that. He won't survive. That sadden me a lot.Actually, to be honest, this incident made me think about my late dad..Was he in that situation when the doctors can't do anything to save him???Ya Allah..I know, that's what we called as faith..but as an ordinary human being, it just keep on playing in my mind. Ya Allah, berikanlah kekuatan kepada diriku yg lemah ini utk menghadapi semua dugaan ini..InsyaAllah, I'll do my best to be a good doctor...Amin..

Another interesting story..I almost fainted when I had my last patient..it happened around 11 pm ...I just don't why suddenly I become like that??? I'm clueless..maybe due the light in the procedure room...or maybe due to the tiredness..or might be due to blood that oozing out like a leaking pipe..hahahaha..i really don't have any idea...hahahaha..Actually, the patient was having epistaxis, so the blood just keep coming out from the nostrils as he is on aspirin for his heart problem and he did expectorate quite huge amount of blood clots..When I told my registrar about that, she thought I just making a joke but when she noticed my face became so pale, she started to worry about myself and asked me to take a deep breathe for couple of times and then asked to lie down...hahahahaha...that's so funny..even the patient himself and his wife also were so concerned about my condition...hahahaha;D

I went home around 11.20 pm...n when I'm just about to take a elevator, I noticed there was 2 texts from Ana...n she said that she'll pick me up...I feel so much grateful for having a friend like her...Ya Allah, thank you so much for giving me such a "wonderful gift" to me ...To Ana, thank you so much sayang..I really..really...appreciate your help...I just can't express how much my gratitude to you...n a special thanks too to Ain, Ana's housemate, for accompanying Ana to pick me up at the hospital at the middle of the night.

Soon, when I arrived at home, my housemates had made me a dinner...Nyum..nyum....I'm so touching...Thank you so much Izzah n Fiqah...Alhamdulillah, I'm surrounded by a lot of good friends..Ya Allah..terima kasih di atas semua pemberian Mu ini, they are another "wonderful gifts " given from You when I reached in Auckland...Daku sangat2 bersyukur dikurniakan kawan2 yg baik dan memahami...Semoga persahabatan kami berpanjangan sehingga ke akhir hayat..Amin...

I think I need to stop here...I want to perform Zuhur prayer..I'm fasting today..puasa ganti...hahahaha...tak tau la nak masak ape utk berbuka nanti...rasa mcm nak masak sambal sardin....nyum..nyum...Alamak, kurang pahala puasa aku..cis...hahahahaha..lol

Miow...

Dear Blog,

Today is my 1st day doing ED run. I was placed in Auckland City Hospital for this 2-week-duration run. Being in the ED for the first time was an awesome experience. I'm so lucky coz I'm the only student for the session so I can jump here n there.kuikui.lol. I mean, I can attach to any on-duty -registrars.

Both of today's registrar are so lovely. They taught me a lot.A credit also should go to the nurses in triage area.They are so helpful.Today, I've got 2 sign-off for my logbook. That's for renal colic n triage scale.Alhamdulillah.I also got opportunity to perform suture.Since this was my 1st time experience it, my hands were so shaking until the registrar, Desmond realized that.But he was so helpful and understanding, he said i've done well for the 1st timer.He also gave me a set of suture appratus for me to keep on practise it. Thanks Desmond!!Really appreciate that.

Tomorrow,I have evening shift which ends at 11.30pm.Errmm..I don't know, how am I getting home..hopefully there's someone will give me a ride..Amin...

Alhamdulillah...General Surgery is over!!!

Dear blog,

Alhamdulillah, I've completed the 2-week-time in catching up the run which I've missed during the bereavement leave. I really feel like wanna to shout to the entire world how relief am I now. This afternoon, I've submitted my POGS and I gave the consultant assessment form to Prof Windsor's secretary. Hopefully, the assessment will be fine.Amin.

As today is the last day, I didn't expect too much since I've completed doing the whole 25 cases in POGs.But luckily, I got such a wonderful opportunity to srub in and participated in the operation theatre. I even had the opportunity to put my fingers in patient's peritoneum to feel for her inguinal ligament and pubic tubercle. I'm so damn happy.yea..yea..Opps..Forgot to mention..the procedure was done for indirect inguinal hernia repair..

Tonight, I want to start to read the new novel which was arrived on last Monday.Thanks Mama n along..really love you guys a lotxxx...uwaaa..i really miss my family...:(

Ok..i think i need to stop here.i'm so hungry n i think my bihun tom yam is ready to be eaten.nyum..nyum...

Fatigue Syndrome??

Dear Blog,

Just wanna drop by for a while. Just wanna express my feeling at the moment. I'm so damn tired.Tired doing surgery in Auckland Hospital.Uwaaa...sob sob..

But I keep on reminding myself to think positively. Although I felt so tired but in fact I gained a lot of experinces and knowledge within this a-week-time. Yeye.*clap clap*

My surgery team is quite large which consists of 5 consultants, 1 fellow, 2 registrars, 2 house-officers and 2 4th year medical students (me and Thalib). Everyday we have more than 30 patients and our ward round are so extensive. Sometimes, we have to do the ward round three times a day. Huhu.

Today, our team has a situation and it involved the patient who being admitted by me and Thalib. I'm so panic at the time.At the end, Alhamdulillah, that's was not our fault. The best thing is, I just realized that I can do things that beyond my thought when there's stressor. Hahaha. I do feel like being a real doctor after that incident. *clap clap*

Ok blog.I need to get adequate rest since my cold and cough getting worst. uwaaa..sedihnyer... bye bye blog...jiha,aja aja fighting!!!

Ya Allah..bantulah hamba mu ini...

Dear Blog,

Tommorrow, I'll be replacing my surgery run which I had missed. Huhuhu. To be frank, I'm kind of scared of getting into new team in Auckland City Hospital (ACH). Before this, I did my surgery run in North Shore Hospital(NSH). But since I have problem with the transportation to NSH, I applied to do it in ACH and Alhamdulillah, it was approved. And although I have missed 3 weeks, I just need to replace for 2 weeks. *clap clap*. Fortunately, one of my batchmates, Thalib Mawjood will be making up these 2 weeks and we'll be in the same team. I'm so glad because at least I'll be accompanied by someone. Alhamdulillah.

Hopefully, I can adapt myself to the new team in short time and perform it up to the expected standard as 4th year medical student.Mudah-mudahan.Amin.

So Najihah, aja aja fighting!!! You can do it!!! *monologue to myself*.lol

Today is Queen's Birthday...which means holiday!!!

Nice meeting you.hahahahah. Oh man,that's so hilarious.hahahahaha.As if i'm talking to someone.ceh.lol.

That's for the intro. I gonna make this blog as my diary since I didn't any for this year. Yeahh!! Actually, the credit should go to Afiqah Ayob, one of my flatmates here. She has a blog and she always keeps her blog updated.Frankly,I become so addicted to read her blog.Fiqah, thanks a lot for being my inspiration *clap clap*.lol --> curik trademark fiqah;-p

So, this morning, it's just out of the blue, the idea came out from my mind. Why don't I start to make one?? Perhaps it's quite worthwhile, I can put anything about my daily activities here. And one of the objectives, is to make my family and friends to be updated with my latest news. So to everyone, "WELCOME TO MY BLOG".lol

Today is Monday but we don't have to go the hospital since it's holiday.lol. Officially, today is the first day I'm fasting in Auckland with Izzah actually. Hahahaha. So, this morning I woke up about 5.20 am and had nasi minyak as my sahur with Izzah.Nyummmm..Nyummm..it was super duper yummy.. For me, it's kind like so prestigious..ewaaahhh..sangat lumayan tu....hahahaha... To be honest, it was yesterday dinner. We had Nasi Minyak delivery from Pakcik Ghulam but honestly, i don't have any idea about who is he??? hahaha. It cost me $8. It may sound a bit expensive but it's really cheap in fact. The portion of that pack of nasi minyak was so huge. I just don't know how to describe it but it's totally banyak ok. The nasi occupied the big tupperware with 2 big drumsticks and mix vege.Nyumm..nyum...alamak dah terliur dah ni..cis, kurang pahala pose ku..hahahaha...

N insyallah..we're going to cook soto for breaking fast this evening. For your information, pose kat sini sgt best because the length is shorter compared to my beloved Malaysia. Subuh is at 5.53am and Maghrib is at 5.11pm. It's shorter right. And the weather is very helpful..It's so chill outside. I don't even feel tired at all. Alhamdulillah.

Yesterday, we went to Avondale Sunday Market with Ana. Ana drove the car.Yeah..thanks ana for give us a ride.muahhxx;D. This market is exactly like Pasar Tani back in Malaysia. Things are so cheap and I bought a lot of fruits here. I bought apples, oranges and persimmons aka pisang kaki. hahahaha. I'll attach the pics later since the pics are in Izzah's handset.

Last Saturday, five of us (Fiqah,Izzah,Fezah,Pu3 and myself) went to Disney High School Musical On Ice. It was held in Vector Arena, which located near to the Downtown Foodtown. It was so happening. Although, I haven't watch both of the movies, I really enjoyed it very much. Here are some pics from that event.
